The Family Advocate is responsible for assisting families with children ages birth - 8 in identifying & accessing resources within Monmouth County.
Visiting Nurse Association Health Group Healthy Families
The Healthy Families Program supports pregnant women or parents/guardians through home visits where a specially trained Family Support Worker brings education and resources to parents and children. Some of these resources include pregnancy, parenting, child development, and nutrition education; other community services include linkages to job training, schooling, child care, and transportation. Home visits last for up to three years.
Visiting Nurse Association Health Group HIV Case Management Services
HIV Case Management provides home and community-based services to individuals living with HIV/AIDS. The program connects persons with medical care and community resources. Referrals to community resources include, but are not limited to, housing, substance abuse treatment, and social services.
Visiting Nurse Association Health Group Mobile Outreach Clinic Program
Services include Registered Nurse (RN) assessments and education as well as care coordination with primary care providers for those who live in transient homes or are physically or mentally disabled.
Visiting Nurse Association Health Group Nurse-Family Partnership
NFP is a home-visitation program for low income first-time pregnant women in Monmouth, Ocean, Middlesex, & Somerset Counties. Home visits continue until the child is two years old.
Visiting Nurse Association Health Group Parents as Teachers
Through home visits, Parent as Teachers helps parents understand child development and connects them to community resources such as insurance, transportation, food, baby items, and health care. Home visits last for up to 2 years.
Visiting Nurse Association Health Group Prevention Resource Network
Prevention Resource Network (PRN) provides high-impact prevention services to assist HIV negative and HIV positive people at risk for getting or transmitting the disease. PRN is funded by the NJ Department of Health, Division of HIV, STD, and TB Services and the NJ Department of Human Services, Division of Mental Health and Addiction Services.
Visiting Nurse Association Health Group Public Health Nursing
Public Health Registered Nurse services are available to those in need of health-related services, including, but not limited to, immunizations, safety, lead poisoning and parenting.
Visiting Nurse Association Health Group School Readiness
The School Readiness program teaches parents to prepare their young children (birth to 5 years) for when they enter school. The learning objectives take place within the community and home setting enhancing parent/child learning within the community culture.
Visiting Nurse Association Health Group School-Based Nurse Practitioner Program
Nurse Practitioners provide primary health care services at the school for children in Pre-K to high school. Services may include diagnosis and treatment of common childhood health issues, medication prescriptions, health screenings, and management of chronic diseases such as asthma and diabetes. Nurse Practitioners also provide health education and referrals to community resources to meet the students' needs.
Visiting Nurse Association Health Group Screenings for Preschoolers
Public health nurses are available to child care and preschool facilities to conduct vision, hearing, and dental screenings to help early childhood centers meet Grow NJ Kids objectives and find and address health concerns early.

VNA nurses provide individualized health & wellness assessments and disease prevention and detection coaching so older adults can focus on living life to their fullest.
Visiting Nurse Association Health Group Special Child Health Services
Special Child Health Services provides case management for children, from birth through 21 years, who have or are at risk for a lifelong physical, developmental, and/or behavioral condition that affects the child's health and well-being. A nurse/social worker case manager assists the family with finding programs and services to help their child.
Visiting Nurse Association Health Group Special Supplemental Nutrition Program for Women, Infants, and Children (WIC)
Women, Infant & Children Supplemental Nutrition (WIC) participants receive nutrition education and counseling, vouchers for the purchase of nutritious food, and referrals for health care and social services. Breastfeeding counseling and support from a lactation consultant are also offered.
VNA Health Group - Early Intervention
Early Intervention is a family training program that helps children (newborn to age 3) meet their developmental milestones. Services include social work, parent-training, speech, occupational, and physical therapies
