Monmouth Medical Center's Community Prevention Program

Crisis Prevention for Children & Adults. Monmouth Medical Center's Community Prevention Program is a service designed to reach clients in the community who may otherwise have barriers to treatment.  Our hope is to reduce the hurdles one might face and make mental health treatment accessible to everyone.

These services are provided at no cost to the consumer.

Service Provided:

  • Mental Health Screenings
  • Return to School Clearances
  • Crisis Prevention Planning
  • Co-occurring specialist consultations
  • Acute Family Support
  • Peer Specialist Support
  • Linkage to Outpatient Providers/Programs
  • Coping Skill Establishment and Review
  • Referral to Specialized County, State, and National Programs
  • Review of Discharge Plans

Appointments available daily!

Ask for a preventative appointment. A clinician will ask some preliminary questions and a timely appointment will be scheduled. Upon arrival, a needs assessment will be conducted by a psychiatric screener in the comfort of the client's own home. Additional specialists can be incorporated for substance abuse, family support, and peer support. An action plan will be individualized for each patient with linkage to services and referrals available. Follow-up appointments are available as well.
