Care management takes you from the first step of getting your family out of crisis and you or your loved one into detox or treatment and following you through all your care needs and placement services.
Care Management is crucial in the process of treatment. After a program is complete it is vital to have aftercare services in place prior to discharge. Our Care Management teams will stay in weekly contact with the individual who has just graduated from an inpatient level of care. We not only stay in contact with the individual but also with their aftercare facility and family members. We offer this on a month-to-month basis or for the year depending on what the individual requests.
